  1.               THE LIVING WILL GUIDE SOFTWARE KIT

  3.      This product is being distributed as shareware.  You are free
  4. to evaluate same for a period of thirty (30) days.  If you make use
  5. of this product after that period, or if you print out and use a
  6. form from this product you are considered a purchaser of this
  7. product and are morally and legally obligated to pay the shareware
  8. fee.

  10.      There are two levels of registration as follows:

  12.      Level 1 consists solely of the forms on this disk which you
  13. may use to prepare your own living will as well as living wills for
  14. your family members.  The registration fee for this level is $9.95
  15. plus applicable sales tax in Florida.  If you wish a new disk of
  16. these forms, please add the appropriate shipping fee ($1.50) and a
  17. duplicate disk will be sent.  I recommend Level 1 to only those
  18. people who are comfortable with legal documents and understand what
  19. they are executing.

  21.      Level 2 registration will entitle you to the book The Complete
  22. Guide to Living Wills.  This book contains detailed information on
  23. completing your own living will, the legal effects of the document,
  24. optional provisions you can add to your own living will, a glossary
  25. of terms and more.  With this book you can prepare a customized
  26. living will reflecting your specific desires.

  28.      The order form included on this disk sets forth the costs for
  29. registration.  With your support I will be able to produce other
  30. forms for your use.  In addition, I welcome comments and
  31. suggestions from all registered users.

  33.      AS ALWAYS, ANY LEGAL DOCUMENT, INCLUDING A LIVING WILL,
  34. REQUIRES A GOOD UNDERSTANDING OF ITS CONTENTS AND POSSIBLE
